Understanding Form N-648 or Medical Certification for Disability Exceptions Evaluations: What They Are and What to Expect

Individuals applying for U.S. citizenship need to demonstrate that they can understand, write, and speak the English language, as well as demonstrate knowledge and understanding of aspects of the U.S. government. The aforementioned English language and civics knowledge requirements can feel daunting and overwhelming for some applicants, particularly when some health conditions make the process more challenging.

In cases where a medical condition prevents an applicant from fulfilling these requirements, the USCIS (U.S. Citizenship and Immigration Services) offers an exception through Form N-648 (Medical Certification for Disability Exceptions) that can offer an important path forward.

What Is an N-648 Waiver Evaluation?

The N-648 waiver is designed to determine if a physical, developmental, or mental health condition truly prevents a person applying for U.S. naturalization from completing the English language and/or civics requirement. The waiver request has to be supported and certified by a professional evaluation conducted by a licensed medical or psychological/neuropsychological professional. The evaluation results are used to fill out Form N-648 and provide evidence supporting the applicant’s request for an exemption based on their condition.

Who Qualifies for an N-648 Waiver?

Applicants may qualify for an N-648 waiver if they have a physical (e.g., traumatic brain injury), developmental (e.g., intellectual disability), or mental health condition (e.g., dementia) that prevents the applicant from completing the English and/or civics requirements.

To qualify, the condition must:

  1. Be diagnosed by a licensed professional.
  2. Impact the applicant’s ability to meet the citizenship requirements.
  3. Be long-lasting (i.e., “has lasted or is expected to last at least 12 months”) and significantly impact functioning. Temporary conditions or lack of education alone does not meet the criteria for the waiver.
  4. Not be related to illegal use of drugs

What to Expect During an N-648 Evaluation

The evaluation process is thorough, as USCIS requires detailed documentation. Here’s what typically happens:

  1. Initial Interview: The process starts with a conversation with the neuropsychologist to discuss your medical history, symptoms, current challenges, and how they impact your daily life. If you have medical records, such as past diagnoses or treatment notes, it is helpful to share them with the neuropsychologist.
  2. Testing: If warranted, the neuropsychologist may ask you to complete a series of cognitive and emotional tests to evaluate areas such as memory, attention/concentration, reasoning, problem-solving, depression, anxiety, and more.
  3. Form Completion: If results of the evaluation confirm that you meet the necessary criteria, the neuropsychologist will complete Form N-648. The form includes a detailed explanation of your condition, its impact on your ability to meet citizenship requirements, and how it aligns with USCIS standards.
  4. Submission to USCIS: You’ll submit the completed form as part of your naturalization application. USCIS will review the application and they will determine if they will grant the waiver based on the information provided. Approval of the exemption is not determined by the neuropsychologist who conducted the evaluation.

Does undergoing the evaluation guarantee that the Neuropsychologist will complete the N-648 form?

No, undergoing an evaluation does not guarantee that the N-648 Form will be completed. The form will only be completed if the Neuropsychologist determines that the applicant meets the legal criteria established by USCIS. If the applicant does not meet the criteria, the form cannot ethically or legally be completed.

Does submitting an N-648 Form to USCIS guarantee that I will receive the exemption?

No, it does not guarantee you will receive the exemption. USCIS officers will review the form and decide whether the exemption will or will not be granted. Even in some cases with a properly completed evaluation and form, USCIS may request additional information.
